An Ohio county prosecutor has opened an investigation into the circumstances surrounding the death of a teenage boy who became trapped by a minivan bench seat.

During the two-minute call with the 911 operator, he gasped, cried repeatedly for help and explained that he was trapped inside his auto in the parking lot of Seven Hills School. The third row bench seat flipped backward, collapsed on him, and trapped him.

The 911 system failed due to either a problem with technology, a mistake by the operator, or a combination of the two, the review found. Hamilton County Coroner Lakshmi Sammarco said he died of asphyxia caused by chest compression. His body was found more than six hours after he first called 911.

He was using voice commands to dial 911, because he didn't have his phone in his hand, said Cincinnati Police Department Lt. Steve Saunders.

The first call ends and the dispatcher alerts local law enforcement.

In a heartbreaking turn of events, which are not fully known, the teenager got trapped in his van's seat which eventually led to his death in a parking lot near the school. Later, a Hamilton County Sheriff's Deputy on school grounds says he checked the van but didn't locate Plush.
